I spoke with a reservation rep. from disney who told me if you dont have medical problems you do not have to send this form in. nor do you need this form at all. Does anyone know this for a fact.. I would hate to be turned away at boarding for not having this form filled out and signed by a physician. noone has medical problems in my family nor do any of us take any medication. Someone please respond we are two weeks away from departure.

Hi, the medical forms are only for those who HAVE medical problems or take medications .
This is done just as a precaution for the guest.
If you had a medical condition or were on certain medications and got hurt, and couldnt speak for yourself, they need to know your problems or they may give you a drug that could have a bad reaction due to the medications you are on or due to your condition you have. It is to protect those of us with medical conditions.

But for someone who is healthy, NO you dont need to fill out a medical form.
Even for those who do fill them out, you either mail or fax them over before the cruise. There is never any mention of it at check in. It is purely voluntary even for those with existing conditions, but they are stupid not to keep the doctor informed of their condition.

Yes this is true. Cruised the first time no medical issues form not needed. Cruises 2nd time needing an accessible room form needed. So no worries if you have no medical problems form is not requested of needed.

What if you're healthy, but do take medications? We are taking along my 65 year-old mother, who takes "maitenance" meds, but is in good health otherwise.

I thought she could just like the medications she takes, but the form requires a doctor's signature. Does she have to make a doctor's appointment just to have him/her list the medications?

Oops - meant "LIST" the medications she takes, not "like".
