We will be in Key West on Monday, October 22 from 11 a.m. to 6 p.m. Fantasy Fest starts October 19. What can we expect other than it being more crowded than usual.

Not much. No official events on Monday until the Leather Fetish party at 9pm.
The stores are really stocked with lots of masks, costumes, t shirts, etc but the crowds won't be that heavy early in the week.

If you've been to Key West before, you probably won't notice much difference during the day on the day you will be there. See this thread which discusses Fantasy Fest:
