About how much should it cost to transfer a family of 4 + luggage between Bangkok airport and city hotel please?

Why don't you get the Montien to pick you up...surely it's part of the service for a 5 star hotel...otherwise you could take the Limo service at the airport ...I'll be there in the morning so will check the price for you...
