Why is it possible for on-line sites and NY Times advertised phone web sites to offer "free" insurance yet I have to pay when I use my agent or directly through the cruise line?
How do they do it?

Somehow I would strongly suspect "FREE" cruise insurance. Check it out carefully before you elect to use it. It could just be a way to find out your personal information and then hack into your financial accounts.

My guess is that it is part of a promo the TA is offering, like an OBC or something like that.

Be very careful before accepting such a deal. There are so many different types of insurance out there with many not allowing you to cancel "for any reason" or have a long list of exclusions that you should read the "free" insurance policy carefully before accepting it.
As the saying goes, "There ain't no such thing as a free lunch" which I've found is a good warning before jumping into a deal.
