Many waiters will ask you if you want to use a Diamond voucher, but it's a good idea to mention it. You can book in advance, but you have to pay for it when you book the reservations, then go through the process of getting a refund on board. Or you can just wait until you are on the ship and pay when you eat.

I can't find it anymore, but I read that the cruise terminal in Baltimore is being used as a staging area for some of the people involved in the work on the port and collapsed bridge.

It may sound odd to you, but when the itinerary changed from Middle East to Africa, some people didn't want to do that, and are leaving the ship to rejoin at some later point. And some of those people are choosing to find ways to visit the missed ports.
