We going on longer cruise, so we almost always leave extra for our cabin steward.

If we have TD, we usually give extra to the waiter and the assistant

They will deal with each individual situation as they occur. I don't think there is a set policy for dealing with such a problem. But generally, they will get you home and they will give you a refund...

This is a case where some days everything will go fine and you will make it. Other days the ship will be a little late or customs will be slow, or traffic will be a mess and you will miss the flight....

Cruise lines will discount fares as they approach sailing IF the sales are slow. They raise fares if the cruise is selling well.
