We celebrated my wife's b'day on our first cruise on DCL. A sign was posted on our stateroom door by our host, and our servers, accompanied by several others, sang Happy Birthday to her at dinner and presented her with a small birthday cake topped by a white chocolate "wave" with an image of the ship on it.
Then, the next night at Palo our server brought her a plate on which Happy Birthday had been elegantly written in chocolate and fruit syrups; inedible, of course, and they misspelled her name, but a very nice gesture regardless. The key to the whole thing is to make sure your reservation is noted with the details - - who, what day, etc. - - and it doesn't hurt to mention it to your servers the first day.

2 weeks ago when we cruised they also gave out birthday buttons to wear. I think at guest services desk and all the CM's make a point of wishing everyone a Happy Birthday.
Kids love all the Happy Birthdays they get.
