Default Notarized permission slip

My TA told me that I need a notarized permission slip from my husband to take my children with me on a cruise because he is not goin with us. How should I word the statement. I am leaving on 9/3/07 from Jacksonville, Florida onboard the Celebration. We will be visiting Key West and Naussa.

I am having problems wording this.

Any advice would be great. Thanks

You are currently married? I don't understand why you would need this. I took my DS on 3 cruises without my husband and it was never a problem.

"Date"

To whom it may concern,

I "insert name here" give my wife "insert name here" my permission to cruise with our children "insert names and ages" on "insert date" aboard "insert ship" traveling to " insert ports of call" and returning to the United States on " insert date".

Should any emergency situation arise I give my wife "insert name" full authority to make any medical decisions or transportation decisions regarding the health and saftey of our children.

It just sounds like they are being overly careful.

Something that he aknowledges you are cruising, and he gives permission for you to have the kids.

Have him sign, you sign, your friendly Notary sign ( and stamp) and you you should have your back side cuvered suffciently.

Mostly, this type of permission is reserved for divorced or separated parents, but I also think is still a good idea for any one traveling out of the country with their children without their spouse.

Many kids "disappear" from vacations with separated or divorced parents.

I am divorced from the father of my kids - so when we took our cruise to Mexico I had a 2 permission slip prepared and he signed and had them notorized. That way both my husband (their step dad) and I could make medical decisions for them out of the country. The US is not so fussy, but Mexico can be.

go to - I believe - familyvacationforum, or do a search, I was able to print out exact forms needed, FYI - RCCL never even glanced at them. But what a sad day should you get an agent that is all about the rules.....

Peace,

Talking permission slip

We too had to get a notarized permission slip. My kids stepmom works for an attorney and she just had them draw up a simple form with information like, my childs name and birthday, where we are going when we will be back and what ports we are stopping at.

It was very simple and then my ex just signed it and they notarized it. Easy.

They have been on cruises before and took the letter and never have been asked for it but I just want to cover all the bases and make sure we are covered.

Look under search for permission slip. There are many threads about this. You can just print one from the search and fill it in. That is what we did for our grandson. Mexico has gotten particular.

Default Permission Slip

I also took my children and it was worded by my ex-wife saying... I so and so give so and so permission to take so and so on Carnival cruise ship from date to date. I also had it notorized but was not even asked about it.

We have discussed this many times on these boards so here is a general summary of what everyone has said in the past. My TA suggested that I have one when taking my kids without my husband. So I did take one.

But since then, it seems like a few years ago (before passports) when traveling to Mexico (since there was a high incidence of parent child abduction), they started to require this letter. But that was back in 2004.

Then with the passport requirements, both parents having to sign it (unless one is dead/or missing), bringing a letter wasn't as necessary. The Carnival documents back in December did say that we should bring a letter from my husband. I didn't notice it on the contract this time.

And in the cases where the other parent is dead or missing, there were other recommended documents to bring (like sole custody papers)
