Well, we have rented many cars in Europe (including in Livorno) and driven thousands of miles in Italy. But, I generally do not recommend renting a car unless you have some experience driving in Europe and Italy. If you do rent a car (all the majors rent at Livorno and there also some good local rental car companies) I recommend you spend the extra few dollars and rent a GPS. Hertz used to deliver right to the pier, but with the security in the port I am not sure of the current situation.

you will get an idea. Do not pass on insurance! With a car you can go to Siena one of the finest (IMHO) of the hill towns (San Gimiano is also available as a close to Florence alternative) Do not however, I repeat, do not, even think of driving into Florence. Even if you manage to find the center of town, you will not find a parking space and if you do . . .
