We are looking at doing a med cruise and I am interested to know if there is much difference in category 3 and 4.Is it purely positioning? If so we may be able to get a category 4 right next door to a category3 if booking early.

We had a cat 3 on our last cruise and we were very happy with it..

Also is there any point in getting a balcony?

Yes - cabins are pretty much the same so just positioning. We have had 3 and 4 on the lower deck. Will do 4 next time. There are no balconies on the RC except for the deluxe suites.
