It seems like all travel insurance medical coverage verbage uses the term "international travel". Our cruise visits St Thomas and San Juan, I asked our usual travel insurance company, Medex, and their position is that i would have to drag my ailing butt back on the ship and wait until we hit international waters before care would be covered. Do other insurance companies make this international/national distinction?

This is probably a quirk of medical-only policies. They are explicitly designed to not replicate domestic coverage.
A normal comprehensive single-trip policy doesn't care if you slip on spilled Starbucks while connecting in O'Hare; as long as you've departed on your trip, you are covered. But obviously those comprehensive policies are much more expensive. SirWired |

Thanks. Travelex seems like an excellent comprehensive policy.

We also use Travelex and they would cover you in San Juan or at any point when your coverage begins.
