There are certainly others, but these are certainly some to look at.

Keith mentioned Rome in Limo which I highly recommend as do thousands of others. Check their reviews on Trip Advisor and this site. However, they will arrange for the private guide to meet you at the Vatican for an additional fee. The driver is not a licensed guide.

Well worth it, IMO.

Are you looking for a private tour that will meet you at the port, take you to Rome and tour the Vatican?

 

Or are you looking for a tour that starts at the Vatican and you will get yourself there?

 

The Vatican offers guided tours but you must arrange it in advance through their official website: http://biglietteriamusei.vatican.va/musei/tickets/do

We need to hear back from the OP; I read the question to mean an actual tour of these locations. The car/driver services do not provide this type of tour unless you pay extra for a guide to accompany you to the sites. This distinction is not always clear to folks new to Italy.

My vote is for romeinlimo. We had Fabio (I call him Fabulous Fabio) and he picked us up in a van, took us thru Rome (they can use the bus lanes), gave us a great tour, and then they arranged to have someone give the Vatican tour. My friends still talk about what a fabulous tour we had.

We just returned and used LivItaly tours. Wonderful. We met early and got in Sistine Chapel prior to opening to the general public. The tour lasted about 4 1/2 hours. We had a private guide for the 4 of us (Luca) and it was worth every penny.
