Yes, I am married to the father of my kids, we all have the same last name. I have taken them out of the country 3 times without him. I was asked for the letter by a customs agent in Freeport on a land trip, was glad I had it

I usually volunteered it at check-in/Customs but I'm not sure anyone actually asked for it. They did look at it briefly though. You can do one yourself. The one I drafted (I'm an attorney) basically said:

 

AUTHORIZATION TO TRAVEL

 

I hereby authorize (full name/Drivers' License) to take (child's full name) on the (Cruise Line/Ship name) during the period of (dates) and to disembark in all ports including (name of ports). (Consider whether to add sentence about medical treatment which may or may not necessary or really enforceable depending on what Divorce Decree, if any, says-It wouldn't hurt to have the Divorce Decree availiable if you are already authorized to make those decisions )

Telephone No/E-mail

 

Affiant/Notary Public Stamp etc
