There's also a Washy-Washy, Happy-Happy, Smiley-Smiley guy on the Pearl! (we were on board 12/2 to 12/9, coincidentally) I loved how he made it "fun" to get your hands sanitized. A lot of passengers I've seen on cruises across the years are averse to having their hands sanitized (why in God's name, I'll never know) but when NCL made this brilliant move, they made it a fun little ritual.

The reason some people may not want to have their hands "sanitized" is that they may have an allergic or other issue with some of the sanitizers.

The other reason could be that they are plain inconsiderate. I always let the "wishy-washy" do their thing.
