Arriving at 230AM

I would contact the hotel directly and let them know what time you expect to arrive.

 

My flight gets in around 10:30, so I suspect by the time I get my luggage and get a taxi into old San Juan it will be close to midnight when I reach the hotel. I plan on contacting them to let them know of my late arrival (although for San Juan, I don't think midnight is considered late, even on a weeknight!).

If I book a room for a Thursday overnight flight to arrive at 230AM, and alert the hotel I will not arrive until Friday morning, would they hold my room so we could check in?

 

Yes, as long as you let them know and chances are you may be prepaying for your room. I would let them know when you book and also give them a reminder call within a day or so of the trip.
