Necessary paperwork for taking grandchildren on a cruise

I know I've read in these boards about the paperwork needed when you are taking children (not your own) on a cruise, but when I used the search nothing came up.

We are taking our 2 teenage grandsons on a cruise for spring break. What do I need to have with us for NCL and any port we may get off at?? Is a notarized statement from the parents sufficient or is there something special we need. Thank you to anyone who can offer help.:)

There is a form on NCL's website to be used when a minor is sailing without any parent or guardian:




Traveling with Minors Unaccompanied by Parent or Legal Guardian*:

The following documents must be supplied at the pier during check-in:




  1. Download and Complete the
    (PDF)Minors or the same parent(s)/legal guardian(s) may be listed on the same form

  2. Copy of both parents/legal guardians Driver’s License or other Government Issued IDAdoptive parents, legal guardians or persons separated or divorced are advised to keep legal and other relevant documents available in order to clarify custody rights.

You may also want to get a notarized statement from their parents authorizing you to make any and all decisions related to medical care while on vacation.
