I know both people in the cabin are required to purchase the deluxe package, my wife has a letter from a doctor saying she can’t consume alcohol due to meds she is on and making her sick, will RCCL allow me to purchase it alone under this circumstance?

Royal will allow your wife to purchase the refreshment package instead of the deluxe package.
