You need to request it, just tell them to call their travel agent or Carnival TA depends how you booked and request a crib.

Just wanted to warn you to be prepared to request it more than once. We just went on a cruise with a little one. We told our TA in advance that we'd need a crib. When we got onboard, no crib. So, we went to the purser's desk and requested one before lunch. Then we told our room steward before the muster drill and dinner. At bedtime, still no crib. So, DH tracked down the first steward he could find in the hall. Our steward brought one soon after. It was troublesome, but worth it in the long run!
