She is a guest of my daughters. So far this is what I have come up with.... I (Insert name of minor child's parent) give permission for (insert name of me and my husband) to take my daughter (minor child's name) on a Carnival Trimuph cruise from July 27 – Aug 3, departure part, Miami going to, Cozumel Mexico, Grand Cayman, Caymen Islands and Ocho Rios, Jamaica. I just have there Mom go to a notary of Public, he has all the forms for permission to travel and Permission for medical care. There Mom has full custody of them, so Know Dad had to sign. I have never had a problem and never been ask for the forms, but I would not travel with out them. I think the Notary of Public cost $20.00 or $25.00. Hope this helps. Go to your bank - THEY have notaries there. Never been charged. Also, I go to my church - one of the ladies in the office is a Notary. That's a good place to look.
