Looks like you have your answer, but to add to the Yes vote, I was asked twice during a July 2017 European cruise/trip from the US to produce my notarized parental consent form for minor travel, because Dad was not traveling with us. The first was during the embarkation process in Rome at the cruise terminal, the second was at customs in the Toronto airport arriving from Rome. Son's last name differs from mine.
Is there a certain form that I need to print up or can it be a written letter that's notarized? Thanks again for all of the advice!

Hello all! Quick question- I will be traveling with my daughter on the Conquest on the 23rd. Me and her dad are not together and she has his last name. Is there a form that he needs to sign granting her permission to go on the cruise? Would it need to be notified?
Any help would be appreciated.
