Answering my own question!

I finally got a response from DePalm Island.  You must book your entrance ticket through your cruise line.  To book any upgrades on the island, send them an email and they will send you the information on how to book an upgrade.  I was able to book a cabana on my lunch break today!

We reserved the smallest cabana and it was $289 on top of the excursion fee from Royal Caribbean.  I know that is pricey, but it is still way cheaper than any cabana on Coco Cay.  And I’m having major back surgery after this cruise, so this was my splurge because I have a long recovery ahead of me.
