Car Rentals, Med Cruise- help please!

Anyone one here have good advice regarding renting cars for the following Med ports: Piraeus, Naples, Civitaveccia, Livorno, Villefranche? My DH and I always rent a car in port and do our own itinerary, going boldly into the world and we always have a blast, getting loads seen on our own schedule. Please let me know your experiences renting cars in any of these cities, I really appreciate your help and look forward to following your good advice!
