I can't speak to food allergies but the water in Grand Cayman is probably cleaner than what you drink at home.

 

Drinking water in Cayman is produced essentially by distilling seawater. It far exceeds minimal standards for safe drinking water.

The water is fine, but if I had a severe allergy to shellfish and were going to any island, I would always carry an epi-pen and benadryl. They cook so much fish and shellfish, would want to protect myself as much as possible to not be dangerous or to not interupt your fun time.
