Tipping a private driver

We will be using a private driver to pick us up at airport in Rome and take us to the hotel. Also sharing a driver to the port at Civitavecchia. We have arranged the price for each trip; does anyone know the general "rules" for tipping these drivers? I guess they are a combination of guide and chauffeur, something I've not used before, so any advice from you experienced cruisers will be much appreciated.
