In Vina Del Mar, there a ton of hotels by the beach, but some are less modern than others. I would recommend staying at the Sheraton or the Casino, which are brand new and should have all the wheel chair access you need. Also, Vina del Mar is a very flat and accessible city, I think it will be very accommodating.

In Santiago I would look at either the Hyatt or the Ritz carlton, have stayed in Both and really enjoyed all the facilities, my dad has walking difficulties but he had no problems in either hotel, for me the Ritz just had the edge over the Hyatt.
