Pre-Boarding Health Questionnaire - Form included!

Ok, if you haven't heard, Royal Caribbean has started (or will likely start soon) requiring a Pre-Boarding Health Questionnaire to keep the H1N1 (Piggies Flu) from boarding ;)

I discovered the form is on the Cruise Lines International Association's website. Although likely the same form Royal Caribbean will use, I can't guarantee it.

 

DO NOT fill out this form 'ahead of time', as it defeats the purpose. Print it out, bring your pen, and fill it out at the port. (You're supposed to fill it out right before you board.)
