I successfully upgraded last week. The email I received provided telephone numbers in USA, UK, Australia and New Zealand as well as the option to reply by email. I called the UK number which connected to a very helpful man in USA.

Travelled there in March. Marella supplied the forms to cabins which were completed by the guests.£15 per person was charged to onboard account. Passport then stamped at the cruise terminal. There is nothing to do in advance of travel.

Cruised there in March. The classic car drivers accepted our US dollars but said pounds and Euros were preferable. For local purchases you can get Cuban Peso at the exchange in the cruise terminal.
