consent forms?

My dau has a Parent/Guardian Consent and Release Form filled in but I am wondering what the Voyage # is. I mean where dio I find this info? I don't see it anywhere on our e documents?

When we completed the form we simply put our booking confirmation number on it. We faxed it off to NCL and then contacted our PCC at NCL asking for confirmation that it had been received and that everything was okay. She emailed me back that it had been received and everything was fine. Never had a problem.
