Parental conscent Form

Does anyone have the Parental conscent form they can post.

It's the form where one parent is going on a cruise and the other is not going.

Thank you! Had been needing one of those for an upcoming cruise with my grandson (a surprise 13th birthday gift) and now I don't have to ask. It is amazing what is available on CC!!

My PVP sent me a form similar to this one but I deleted it because I knew I would not need, but come to find out I should have kept it because now my niece needs it.

 

Thank God for CC..... got the form and its ready to be signed. :-)
