Illusionarium on night 1 - included in UDP or not ?

It is no longer included. I am not sure when the changes comes into effect, but it will be so for our Jan '16 cruise. Per my PCC, there will however be a 20% discount on the Illusionarium. Unfortunately, I cannot remember if this is a UDP perk, or a Haven one.

Give NCL a call on Monday see if bookings have opened up they can book it for you.

I booked our upcoming (June 6th) cruise at the end of March, and was told by an NCL rep on live chat that it was no longer included free. When we made our reservations we had to pay the regular price but I'm still hearing that some people are getting it for free. I noticed that they changed the UDP terms a while back and it currently shows no discount for paid shows for sailing before July 3rd this year and a 20% discount for sailings after July 3rd. Kind of upsets me that some are still getting included for free on current sailings

It upsets me as well. I really had to fight for it. Because of my ordeal I booked my first cruise on another cruise line. NCL is showing no respect or compassion for their cruisers. I have my cruise on June 28th I hope they pull it together by then.
