MDR table number

Will our MDR table assignment be noted on our ship card when we board, or will it be on a note card in our cabin?

Note card in your cabin. Then you can go to the lounge near the dining room (usually) and look at the dining room "map" to see where it is, if you want. Or just wait until your dining time and they will show you to your table. (Either way, the first night, they usually show you to your table.)

Are traditional dining time guests seated on the main floor of the MDR and the select dining guests up on the second level?

Well, for the cruises we have been on...select dining is on the upper level...and they have half the space up there dedicated for this seating. Lots of 2 toppers..that can be combined into 4 or larger if needed.
