Can someone explain specialty dining pricing to me?

First time cruiser sailing on Allure in August, and I'm confused about the pricing structure of the specialty restaurants. Some (for example, the Izumi Hibachi Experience) have what appears to be a flat rate (Izumi says "Prices starting from $45 pp) but then certain menu items also have a price next to them as well, while others do not.

 

For a family of 4 would it be $180 PLUS any "extras" we ordered? I assume drinks are extra as well if we don't have the drink package, but what about sparkling water, soda, coffee, etc.?

Sparkling water and soda would be extra.

 

Int he case of Izumi, the fixed price is for the hibachi dinner, which would likely cover the entire dinner. The a la carte prices are generally for those not eating the hibachi dinner.
