Are the balconies larger on Royal Class ship cabins  D148 and up as shown on deck plans ?

Cabin D148 is a Deluxe Balcony class DE.  The room is about 1 sq. M. (11 sq.ft.) larger that a standard balcony.  There is room for a loveseat by the patio door.  But the balcony is. No larger than any balcony on the deck about 120 cm (4 ft) deep.

See attached.

BTW: The D148 has a door connecting to the next cabin and has a 3rd Pullman bed (from ceiling)

It depends on the ship. Royal and Regal have standard sized balconies in that part of the ship, as per the deck plan above. Majestic and Sky appear to have much larger balconies.

Of course you pay a premium for those cabins as they are classed as Premium Deluxe Balconies. We had D424 on Majestic which is just a standard BA cabin with a much larger balcony at no extra cost.
