we leave this Monday on Disney Dream, when we disembark, we have a car reservation at the airport. Just found out that Enterprise no longer has a shuttle going to the airport. Can I catch a shuttle (with fee) to the car rental the day of? or do I need to reserve one ahead of time? Family of 4 and 2 car seats age.

Grab a taxi to the airport - van cabs are the same fare as the standard cabs - should be about $30 on the meter.  Shuttles will charge per person and will wind up costing more and will likely drop you at the departures part of MIA, not the car rental center.

I see that shuttle will cost 40, does cab requires car seats?

Yes, as does any shuttle with seat belts. Tip the driver a bit extra for the time spent installing and uninstalling your car seats.
