Need HAL Medical Records Email

I paid several visits to medical on our last cruise, and now we need medical records.

 

The old email that I've used for years bounced back.

I received the medical records release from HAL Guest Relations. This is the email address that I was asked to send the completed form to: mrecordsrequests@carnival.com.

Not sure if you can request the blank form from them.

Thanks, mine was with a HAL extension, but that was pre-pandemic.

 

I guess Carnival must be handling it all.

 

I did receive the bills onboard, but that's just a supply list with prices, and we can still see a lot from it, including the covid tests.
