Boston - airport to pier

I know this is going to sound like I'm belabouring a point, but, like the original poster, I'm just trying to figure out costs for an upcoming cruise.

 

Our itinerary is North Atlantic and Europe, but does not have any US ports, so we have no need to bring US$ (we are flying into Boston from Canada) except to pay for our taxi from the airport to the cruise terminal and vice versa.

 

Could someone please answer a couple of questions for me:

 

1) I read the following on the Massport website: "All fares leaving Logan Airport are charged a $2.00 airport fee. Travel through the harbor tunnels will cost an additional $4.50 toll fee."

Are these charges included in the $25-30 estimate of the cost of the taxi?

 

2) Although we are very experienced cruisers, we never use taxis at home and most of our travel is in Europe, so we don't know about expected tipping in the US.

I've read that 15% is about the usual amount to tip a taxi driver, but are you expected to tip more than 15% of the fare when you have a lot of luggage? If so, how much per bag?
