That's going to be wickedly expensive, it's about 350 miles and you'd pay for the round trip, because the driver has to get back. There's also a limit to how long they can drive each day so you'd likely also have to pay for an overnight stay for the driver. This would all be included in the price so you wouldn't necessarily see it itemized, but it will add up.

Why on earth would you want to drive when there is a smart way to get there by high speed train with only one change of train en route at Pisa Centrale . (Do NOT go via Roma Termini )

 

For example Weekdays : Civitavecchia Dep 1047 Pisa Centrale arr 1259 Change trains

Pisa Centrale depart 1332 Milano Centrale ar 1645 .

 

Book on line via Trenitalia website, but book as two separate legs, compulsory seat reservation . The high speed Trenitalia Freccabianca trains are an absolute joy to travel on .

Wickedly expensive is probably an understatement :). But to answer the question, why use a car (our favorite mode of transportation) it is often a lot easier then using the train....if one has a lot of luggage. As we constantly warn here on CC, if you plan on training your way through Europe you must be capable of handling your own luggage (including moving it some distance, lifting it on trains, protecting it, etc). We also like a car because it lets us access places outside of cities. But in Italy (and most of the World) you do not want a car to get around cities.

 

And by the way, using a plane from Civitavecchia to Milan would be sheer madness and make no sense in terms of logistics and cost. We would suggest that your best options are either the train or a rental car (which you drive).
