To those who dismiss it as "just an accident" -

When that attitude permeates through every level of an organisation what do you get? Costa Concordia.

You are comparing apples and watermelons! We are discussing the closure of a fire door, not a Captain that was absent from the bridge.

 

It was just an accident that the OP should have handled on the ship. He can contact corporate now, but it should have been dealt with on the ship so it could have been investigated immediately.
