Claim must first be rejected by your primary carrier. This can be frustrating. Came down with flu in the Caribbean and spent six months begging my blue cross carrier to reject claim. Had to file three times before they finally turned it down. Was then able to file with travel insurer who paid off one week. This is why we pay a bit more and get insurance that is primary. I don't want to have to file a claim and wait for the rejection and then submit the claim to the travel insurance company.
