Spanish Health Forms...

I know Spain doesn't require Covid test for fully vaxxed & boosted travelers entering the country, but it seems they still require a health form pre-entry. At least that's what their travel website still says. Anyone have any current experience with this form? The rules are different if coming by sea according to the website. I am asking ONLY about airport entry. TIA.

I am going to Spain next week prior to my Prima TA. I completed the form on line up until the part I can do 72 hours before arriving. They will email me at that time to finish and I will get a QR code for entry. Very similar to forms I did last year for Greece and Italy. I believe you will need to show the QR code to board your flight.

Thanks. I checked there first and there was an older post re the form being necessary but I wanted a current take on it since the "no Covid test" changes. I read the rules on the website before I posted my ? but I wanted some real person experience as things are ever changing.

Thanks. That was what I was reading on the Spanish Travel Health site but I wanted a first person report. I will fill mine out before my TA in Nov. As I have a transit thru Munich, I may not need it as I will be traveling from one EU county to another EU country but best to be prepared.

Just completed 7-Day Epic Med out of Barcelona. Our entry in to Spain from US required the form and QR code from the SPTH website/app.  But since you are traveling from Munich to Spain that may not apply due the requirement below. The form on the app was easy and just required vaccination information.  No COVID tests were required.
