trying to sign up for Voyagers club, won't work!!

Trying to sign up someone to the Voyagers club. All info is correct but it says booking does not exist.

 

There is also a drop down box with all these different countries and the last one says United States with cruises to Cuba.  Not sure what they want you to enter for that

 

Any help would be appreciated.
