Passport for Disney Cruise

Hello, Need help. Im taking my niece on the disney cruise with my daughter. She needs a passport and her mother is the only one listed on the birth certificate. Does she need to fill out the consent form for the father and have it notorized ?

What form do I need to travel with my niece ?

If her mother's name is the only one on the BC, bring a certified copy of that, and the notarized permission letter from mom.

If you niece is under 16, she and her mom will need to appear in person at the passport acceptance facility to apply for a passport.

Your notarized permission letter from the mother should include authorization to make health care decisions on her behalf if the need arises.
