declaration form and medical forms

In Port of Miami last week, there were no forms. When we went to the agent to get our sign & sail cards he said "And I have to ask you this. Is anyone in your party feeling sick?"

Now, I realized that this was a terminal that had no kiosks, and it was all full service at the agent's counter, so it "might" be different in terminals where there are kiosks and all you do is "get" your key from the agent.

do anybody know where i can get a sample of the medical forms you have to fill out in prior to getting ship...so we need to stay in line filling out forms...havent cruise in a year i did see them once before online and and got them..
