Then we mail a photocopy of the financial statement showing we own it WITH a letter that shows your name, the booking number, sail date and ship to: CARNIVAL CRUISE LINES Research Supervisor Reservation Administration 3655 N.W. 87th Avenue Miami, FL 33178 Tel 800-438-6744 ext. 70041 Fax 305-406-5882 The OBC usually shows up on our documents (we still get them snail mail). It's not easy to find. From Carnival's main web page, click on Investor Relations (bottom right, under About Carnival). At the top of the next page, click on Investor Relations. On the left-hand menu you'll see Shareholder Benefit (third from bottom). This takes you to a page with Click here to view our Shareholder Benefit. I sent my documents in via regular mail, then about 2 weeks later called Carnival directly and asked if they had received my "Shareholder Benefit" documents.
