1. Contact NCL or your travel agent and ask that the Visa be added to your reservation.  The $75 pp will be added to your onboard account and you will receive the visa/tourist card when you check in for your cruise.

2.  The OFAC certification form is a US requirement.  People to People is the category, but I believe most now check "support of Cuban people".  Ask NCL when you contact them for the Shore Excursions.

3. Call Shore Excursions 1-866-625-1167 to book your NCL shore excursion.  It will be charged to your onboard account.  And the $50 credit will be applied onboard after you have taken the excursion.

4. Try the access desk for the CPAP machine - accessdesk@ncl.com

RE the Shore Excursion credit

When you phone  1-866-625-1167 they will know that you have the "perk" but to make sure say that you want to RESERVE shorex.  Then all the ones you RESERVE will appear as "full" debit charges on your folio the first night on board.  The $50 will be credited to your folio account overnight on the day AFTER you have completed the shorex.  Remember $50 per CABIN and ONLY 1 per PORT.

Any Latitudes discount Should appear automatically on the initial folio charge, check with Shorex on board that it was.  You can have both the Free at Sea and Distinctive Cruise "Perks" so have $100 instead of $50, same rules.
