Different name on vaccine card from drivers licence

All documents need to match or there needs to be documentation as to why they do not (for example if your name has changed and that is why the name on your birth certificate isn't the same). So assuming the people at the pier do their job, yes she will have a problem.

If the DL and passport have the correct name, it is an easy matter to change the vaccine card.  I suspect it is the other way around.  In that case I like  Purvis post #3.  

 

Whatever you read here, I would hope you would base your decision on a discussion with the cruise line.

You could have your granddaughter check with her State's health department as they usually have a centralized immunization system. She could get them verification of the name change and they will update her record(s). She should then be able to print out the verification of her Covid vaccine(s) in her new name. That letter is acceptable in lieu of the vaccine card.
