There aren't ay casinos within walking distance. The closest casino is at the Sheraton Puerto Rico Resort and Casino, a short taxi or Uber ride away. Note that this is NOT the Sheraton Old San Juan that is across the street from the Old San Juan cruise piers. It's a separate Sheraton hotel near the convention center about 2 1/2 miles away.

If you want some old school gaming the casino in the Fairmont El San Juan is a favorite-- wood paneling, chandeliers, etc. The adjacent Chandelier Bar as you walk is was always a good time. You'll need to Uber from old town like you would for any casino. 

 

The casino at the Ritz Carlton right down the street was also a favorite but that hotel still has not reopened and doesn't look like it will any time soon.

Now that the casino is under the Foxwoods banner I wonder if it's changed a lot from what it used to be.

I saw that. At very least its still in the same space and it looks pretty much the same based on the website. Hopefully they didn't mess with it too much-- that was quite a scene back in the day.

There is a casino at La Concha Renaissance.  You would have to taxi or uber there, but not far.      I just stayed there for a week.  They had $10 blackjack tables in the middle of the day.    Down the beach there was also a casino at the Marriott Stellaris.  Probably a similar drive timewise to the Sheraton as well.
