I have read some say it is with the docs.. some say it is not.. you can get it at the Port.. and it only takes a couple secs to fill out.. don't worry:)

If you are going to the Bahamas, yes, you are supposed to fill it out and turn it in. We were given one to fill out in the terminal in Miami in June; it didn't come with our docs. We will be on the Sensation in about a week and a half and have e-docs, so I imagine we will be completing them in the terminal again.

Our first cruise, it came with the docs. Our second it didn't. They were handing them out as we entered the terminal and we took a couple of minutes to fill them out and we were good to go. Not a big deal.

I filled out four of them and was never asked for it at all.

Downloaded edocs and got paper docs within past few days. Form is not included. We've always gotten them in the port and filled them out in line.

As soon as you go thru the first checkpoint (to check for boarding passes) they will hand them to you and you fill them out before you get to the registration desk.
