2nd health form that you get 72 hours before cruise

sI am driving across country and was wondering if this 72 hour health form is filled out and sent to Carnival or if you print it out and take it with you. I am somewhat nervous as I am not a techno person. I still have a flip phone. Also, are these forms available at the terminal if you dont have one. We are cruising out of Seattle. Much Thanks Ya'll.

Are you stopping at a hotel at all? Check if they have a business center. I'm sure someone at the hotel could help if you needed it.

I would wager that it couldn't hurt to bring a printed copy after submitting it electronically.
