This is Spanish Governments official website.

It says that you only need a certificate of vaccination, certificate of recovery or certificate of negative test results.

This is true ,but Spain does not accept a U.S Certificate and needs a digital Cert Anyway it costs nothing to fill out the form . you might as well have it !! then you don't have to worry either way.

As of Tuesday 20 September 2022, the Spain Travel Health (SpTH) portal, both the website and the mobile applications, will no longer be operational. It will NO LONGER be necessary to fill in the health control form to travel to Spain NOR to present the SpTH QR code when boarding or at the arrival airport.https://www.sanidad.gob.es/en/profesionales/saludPublica/ccayes/alertasActual/nCov/spth.htm

Thanks, will be flying to BCN for a Viking cruise.  They removed the Spain form from their pre-embarkation requirements doc.

I can't find what is considered as "proof" of vaccination for US citizens as it seems to discuss EU digital certificates & exceptions for some non-EU countries, but the US is not listed. Am I missing it? My proof is my CDC card. TIA

Yes, CDC card is your proof.

Are we still required to fill out the said authorization when FLYING into Spain? I know Covid testing isn't required if we are fully vaccinated.

As of October 21st, all Spanish entry restrictions had been lifted (as related to the topic of the original post).

It would be interesting to see when/whether/how cruise lines sailing from Spain react to that.
